Denver Health Clinic School-Based

Denver Health is pleased to be a part of the South High community. As one of eighteen School-Based Health Centers (SBHCs) throughout Denver Public Schools, we offer comprehensive primary care (routine checkups, physical exams, mental health counseling, and health education) for DPS students. Our health services are provided at NO COST to all DPS families!

Parents may complete paper consent forms available at the clinic or complete the forms online   

Students can make appointments for their physicals at South by calling the clinic any weekday at 720-423-6260. Our hours are 8 AM to 4 PM Monday through Friday.   Remember that annual physical appointments need to be at least one year after your last physical.  We are happy to provide documentation of your last Denver Health physical if you have a current sports clearance.  Unfortunately we are unable to provide sports clearance documentation for other practices such as Children’s Hospital, Cherry Creek Pediatrics, Rose Pediatrics, Kids First, etc.

Additional appointments at other school-based health centers are available by calling our central line at 303-602-8958.

In order to utilize SBHC services, students are required to have a parent or guardian complete a consent packet. Register now with our convenient eConsent (Students under 18 must have a parent or guardian fill out the eConsent, students 18 and over can fill it out themselves. Once you have completed the eConsent, call or email us to setup an appointment.

New consent packets are required to be signed each school year and once students are consented, they can easily access services.
